Steps: 1. Print out this form. 2. Fill in the printed form carefully. 3. Fill in your mailing address which will not be changed within at least one year, so that notice of winning will be mailed to you from the State Department (not from this law office!). 4. Sign your customary name using your native language under " Signature". 5. Get a copy of your recent (passport photo size) photo ready. 6. Get ready an international money order of $50 for one applicant or $80 for two applicants plus $5 if you request a Certificate of Mailing from a U. S. postal office. 7.
