5. Copy of your Social Security Card or Social Security Number;
6. Divorce document or other marriage termination paper, if you were married before;
7. Complete affidavit of support with evidences, such as employment verification letter from your current employer, W-2 form AND tax returns for three years, or other assets, the name of persons you previously provided financial support for and the dates; current household members who are living with you and depending on you.
8. List of addresses and the time period you have lived during the past five years;
9. List of the employers and their addresses you have worked for during the past five years; your job title and the time period for each job;
10. List of children’s name, gender, date of birth, country of birth, if any;
11. List of your parents’ names, birth date, country of birth, current city of residence;
12. If have filed any immigrant petition(s) for any one before, give the beneficiary's name(s), date of filing and the result.
13. If signed Affidavit of Support for anyone before, give the alien(s) beneficiary(ies)' name, birth date and date of signing the affidavit.

5. Copy of every page of your passport if in the US;
6. Copy of your I-94, Visa, and Social Security Card (if you have one);
