The following documents are required for a B visa issuance:

1. Form OF-156, filled every applicable field.  Marked "N/A" where inapplicable to the applicant.

2. The applicant's valid passport.

3. Supporting documents showing his/her intent to stay in the United States temporarily and the intent to return the home country after a short visit.

4. Application fees( it varies in different countries.)
