Read this page very carefully! Do not send two applications/entries for one applicant. Only ONE entry form will be submitted by or for each applicant during the registration period. Submission of more than one entry will disqualify the person. Do sign, personally, the entry, preferably in your native alphabet. Failure of the applicant to personally sign your own entry will result in disqualification. Do use the fast way to deliver your application to us. BUT do not use registered mail, certified mail or express mail or other private carrier to deliver your application directly to the visa process center. The State Department states explicitly: use of express or priority mail, fax, hand, messenger, or any means requiring receipts or special handling will not be processed! Do let us to take care of your application. Do not try to deliver your application before or after these dates designated by the State Department. Your application will be disqualified regardless of when they are postmarked. Do send your application to us and trust us with your application. Do not mail your application to the address you used last time. The center processing your application has moved to another location in another state. Please NOTE the application sent to an incorrect address will be disqualified. Do send us a recent 1.5 inches (37 mm) square in size photograph of the applicant, preferably less than 6 months old. Do not use staples or paperclips to hold the photo. The photograph (not a photocopy) should be attached to the entry with clear tape, or simply mail the photo in a small envelop -- let us take care of it! Warning: You will be disqualified for ANY
