1. Birth Certificate;
2. Citizenship Certificate if naturalized;
3. Copy of your Social Security Card or social security number;
4. Your current address;
5. Your parents' name, date of birth and current residence.

Document required from the alien relatives:

1. Notarized birth certificate with English translation, if not in English;
2. Your alien relative's name, current address, other names and birthplace;
3. List of the alien relative's children and spouse’s name, date of birth, country of birth.
